APPLICATION CHECKLIST

A complete application for admission includes the following materials. You are responsible for completing and assembling the application package.  GMAT and TOEFL scores will be sent directly to our office by ETS. Our code is 79K-2S-27. Other materials should be sent to:

Graduate Application for Admission*

Complete personal data and information on your intended area of study, education, and work history. Online application available here.

Application Fee*

After you submit your application you will be asked to choose a form of payment. If you pay by check or money order, submit the check or money order for $56 made payable to the University of Wisconsin-Madison to the Evening MBA Office at the address above. Admission decisions will not be communicated to the applicant until the fee is paid.

Record of Residence*

This form is filed with the University Registrar's Office for all university students and is used to establish residency for data collection purposes only in the Evening MBA program. Evening MBA tuition is the same for residents and non-residents of Wisconsin. Review section 36.27(2) of the Wisconsin statutes. If you claim Wisconsin residency for data purposes you need to complete the record of residency forms. For statutes governing residency for tuition purposes, see section 36.27(2) of Wisconsin Statutes.

Essays* (2)

The Evening MBA application requires two essays.

  • Describe your reasons for graduate study, why the lock-step cohort program is a good fit for you, and why the general management major is a good fit with your career goals.
  • Describe a professional situation in which your personal values were challenged by the way business was being conducted. How did you resolve the situation?
  • Upload your essays when you submit the application.

Resume*

Submit a resume outlining your academic achievements and business experience. To assist in the review process, please indicate the type of firm and scope of operations for each organization for which you were employed. Upload your resume when you submit the application.

Copy of H1-B visa or Permanent Resident Status and letter stating your intentions to continue working while in the program. (International applicants only)

Evening MBA applicants must have either a H1-B visa or Permanent Resident status. No student visas are issued to Evening MBA students. Submit a copy of the H1-B visa or Permanent Resident documentation, and a signed statement indicating your intent to continue working while in the Evening MBA program.

Recommendations* (2)

Select references that will provide detailed information in response to the questions on the recommendation form. At least one recommendation must be professional. Online recommendations are offered through the online application. Provide references' email addresses.

Evaluators submitting the recommendation form on paper should send the original copy directly to our office at the address above. Send the recommendations to the address provided above. Download recommendation forms.

Official Transcripts/Academic Records

You must submit official hard copy transcripts/academic records, which identify the degree, major and courses completed from each educational institution attended since high school. If your degree and date awarded do not appear on your transcripts/academic record, submit an official certificate of the degree awarded. Records must have a seal and signature in ink from the institution's authorized official, such as the registrar or recorder of records. For degrees from international institutions, records must be in the original language of the institution attended, and accompanied by official English translations. Have transcripts sent directly to the Evening MBA program from your schools to the address listed above. You may upload copies of your transcripts when you submit your application and these can be used by the admissions committee for review, however you must submit original hard copies sent directly from your schools in order to enroll.

Graduate Management Admissions Test (GMAT)

Please direct the test administrator to forward your test results to the University of Wisconsin-Madison (institution code: 79K-2S-27). The test must have been taken within five years of the term for which you are applying. For more information on the GMAT, call 1-800-462-8669 (U.S. and Canada only) or go to http://www.mba.com.

Test of English as a Foreign Language (TOEFL) score (only if native language is not English)

If English is not your native language, please direct the test administrator to forward your test results, taken within two years of your intended start term, to the University of Wisconsin-Madison (institution code: 79K-2S-27). International applicants with undergraduate degrees from U.S. universities may contact the Evening MBA admissions office to request a waiver of the TOEFL. More information on the TOEFL.
